Please note that the Print Option form is no longer available after certification.
However, you may now Print the 'Confirmation of Filing' form which is accessible from the
Broker Report after the filing has been accepted by NSLA.

To print the 'Confirmation of Filing' form:
  • 1. Run a broker report with the Print Preview or Screen options.
  • 2. All affidavits that have been accepted by NSLA will have the Insured Name highlighted.
  • 3. Click on the Insured Name link to view and print the 'Confirmation of Filing' page.
