Online Annual Tax Filing Instructions
The 2009 Annual Tax Statement will be available online in mid January, 2010. To view/print a copy of the transactions (broker report) go to “get broker report” and enter the dates for the year. Please do not send us a copy of your broker report.
Review the form for accuracy. If you feel there is a discrepancy please call our office (775) 826-7898 or 1-888-334-4577.
After completing the annual statement, follow the instructions on the form to submit it electronically. If you are making a payment with your annual statement, please make your check payable to Nevada Division of Insurance. Make sure the surplus lines license number is noted on the check and mail to: Nevada Division of Insurance, 788 Fairview Drive, Suite 300, Carson City, NV 89701-5491. Once your payment is recorded, you will be able to print a confirmation for your records. The electronic form submission and payment are due March 1, 2010. If a payment is due the submission will not be considered complete until the payment is received.
NOTE: If there are any open quarterly tax statements, you will not be able to submit the annual statement until those quarterly forms have been completed and submitted.
If a payment is being made electronically using the Automated Clearing House (ACH) please send us confirmation of payment. Information regarding ACH can be found on www.doi.state.nv.us
